MFJC Yizkor Book of Remembrance 5785 / 2024-25 Listing Request

 
It is our tradition at Mt. Freedom Jewish Center to prepare a
Book of Remembrance for distribution during the Yizkor service on
Yom Kippur.  It is considered an honor to the memories of both the
recently deceased and other family members to include their names
along with a contribution.  We suggest a minimum donation of 
$10 per loved one listed in our Book of Remembrance.

This year's Book of Remembrance will list your loved ones' names that were included in last year's Book.

If you did not participate last year or would like to add additional names to last year's listing, please add them in the fields below.

In total, a $10 donation for each loved one, previously or newly listed will be applied to your account. 

This form submission will request donation payment for all the listed loved ones you indicate (prior and new). If you have any questions please contact the shul office at office@mtfjc.org.

Please indicate any changes you wish to make to existing listings as well.
